Acknowledgement of Lodging (Four Night Minimum Requirement) and Payment Information*
By completing this form, I agree that my information will be used in order to register for the BPA National Leadership Conference (NLC) in Chicago, Illinois.


As part of my NLC registration, I am required to stay four nights at the conference property and will be responsible for all registration fees ($125), travel expenses, meals, and hotel fees and taxes.


Additionally, I understand that if I am under the age of 18 years old, I must be companied by my legal guardian or an individual approved by my legal guardian. In order for conference guests to attend all aspects of the NLC, they must be registered as a Chaperone/Guest ($90)

I also understand that my conference registration must be paid by April 15th, 2024 and that lodging will be paid before attending the NLC.
